Flew trials on the 02 July 1973 to determine single engine absolute ceiling performance in the PNG area.

At Six Mile Reservoir near Noosa Qld on 14 April 1993 the crew was conducting a STOL trainer when it impacted with a tree damaging the left hand wing.
- A4-228 was the first RAAF Caribou to be placed into long term storage at Oakey QLD on the 13 May 2009.
